Pro Function Sports Injury Clinic has been selected to
provide Concussion Baseline Testing for the Elgin-Middlesex Chiefs. Beginning August 9, each team has been
scheduled for a session at the clinic, located at Centrefield Sports. Details, including registration link and
dates and times are noted below.
Parents must register each player online prior to arriving for their session. You should have received an email with the registration link; or click here to register now: Registration
After you have completed your registration you will receive a unique ID number for your account. You will need to bring this number with you on the date of your test in order to complete the baseline testing.
Note: if you are a parent of more than one player you will you need to register a separate account for each player.
Please note team testing dates below. If you cannot attend on your scheduled date, please contact Chad Lambourn, Chiefs Equipment Manager, at [email protected] to make alternate arrangements.
